Two-way
Switches between two outlets, the overhead head and the handset. Found on simpler concealed shower installations and on the great majority of exposed bar mixers.
Three-way
Switches between three outlets. Often overhead, handset, and either a bath spout or a body jet, with each position clicking firmly home on the trim plate. Found on multifunction concealed valves.
Both variants are stocked. The two are not interchangeable, even when they look similar from the side. Position count on the existing control is the safest cue, because the cartridge inside has to match the number of click positions on the trim handle.

Do I also need the extension piece that goes with this cartridge?
Sometimes. A diverter extension is a separate part that lengthens the spindle of the cartridge to reach through deeper-mounted trim plates. If the existing valve sits flush behind the wall and the trim plate fits without packing, no extension is needed. If the previous install used an extension or a longer spindle, the replacement cartridge will need one too.
